Registration Form
Swan Lake First Nation will be conducting a vote of the membership to ratify the Land Code and the Individual Agreement which will be provided to you by the Lands Office. The vote has tentatively been set to take place at the Swan Lake Band Office on September 9, 2010, in Brandon on September 10, 2010, in Winnipeg on September 11, 2010 and the final vote day at the Swan Lake Band Office on September 13, 2010.
The Notice of Vote will be distributed soon.
As a member of Swan Lake First Nation, you are entitled to vote provided you will be at least 18 years of age on the date of the vote and have registered with the Ratification Officer. To assist you in the registering process, please find enclosed:
- A Registration Card
- An addressed and postage paid envelope

Carefully follow the steps outlined in the instructions to ensure that you are registered in order to receive your ballot on the day of the vote or for prior if your intention is to mail-in your ballot.
The Voter Registration Form may also be hand delivered to the Band Office. The Ratification Officer will also accept registration forms that are faxed to (204) 943-4242, scanned signed copy e-mailed to bjohnston@darcydeacon.com or hand delivered at the polling or advance polling locations. The Ratification Officer must receive the registration form for your vote to be counted.
Please be aware that missing information or the failure to provide a witness signature will result in your exclusion from the vote until such time as your form is completed in full. Any Voter Registration Cards received after the close of the poll on September 13th, 2010 will not be accepted.
Once the Ratification Officer has received your Registration Card and following the posting of the Notice of Vote, a ballot package will be sent to your mailing address that you provided on the form if you have checked that you wish to receive a mail-in ballot. For those that will be able to attend one of the four voting dates, a ballot will be held for you at the polling station until you arrive.
